Mexico Academy & Central School

The application for employment certificate is available digitally for download on this page. Please have a parent or guardian fill out Part I (marked in purple on the first page) and then a physician fill out the Physical Fitness Certification (marked in purple on the second page).

Once those two parts of the application are complete, please return the form in one of the two following ways:

1) Email the form to Lisa Wood (lwood@mexicocsd.org)

OR

2) Mail a physical copy to:
Mexico High School, Main Office
3338 Main Street
Mexico, NY 13114

Both inboxes will be checked regularly to complete the employment cards.
